About

My name is Nicole Falconer, LMSW, SAP

Welcome! I’m Nicole Falconer, a licensed master social worker based in Muskegon, MI, with a deep passion for helping individuals find healing and reclaim their lives from substance use disorders.

I hold a Bachelor’s in Social Work from Eastern Michigan University (2011) and a Master’s from Grand Valley State University (2014). Over the years, I’ve built a career rooted in compassion, advocacy, and accountability—working in diverse mental health and recovery settings including Pine Rest Psychiatric Hospital and HealthWest. Currently, I serve as a case manager for the Muskegon County Recovery Court, supporting individuals navigating the criminal justice system as they work toward stability and recovery.

In my private practice, I specialize in SAP (Substance Abuse Professional) assessments for Department of Transportation (DOT) employees. With SAP certification through NAADAC and ongoing work toward my CAADC credential, I’m committed to providing thorough, fair, and supportive evaluations. My goal is to help individuals who have tested positive for substances return to safety-sensitive work roles equipped with greater insight, new tools, and a stronger foundation for lasting wellness.

Outside of work, I enjoy life in Muskegon with my family and two cats, and I find joy in caring for my houseplants. Since 2022, I’ve also had the honor of serving on the board of Life Align, INC—a nonprofit dedicated to uplifting those in recovery.

No matter where someone is on their journey, I believe in their ability to grow, heal, and thrive. If you’re looking for compassionate, evidence-based support through the SAP process, I’m here to help.
