About

My name is Nicole Falconer, LMSW, CAADC, SAP

Welcome! I’m Nicole Falconer, a Licensed Master Social Worker based in Muskegon, Michigan, with a deep passion for helping individuals heal and reclaim their lives from substance use disorders.

I earned my Bachelor of Social Work from Eastern Michigan University in 2011 and my Master of Social Work from Grand Valley State University in 2014. Throughout my career, I’ve remained grounded in compassion, advocacy, and accountability. I’ve had the opportunity to work in a variety of mental health and recovery-focused settings, including Pine Rest Psychiatric Hospital and HealthWest (Muskegon’s Community Mental Health).

Currently, I serve as a case manager for the Muskegon County Recovery Court, where I support individuals involved in the criminal justice system as they work toward stability and long-term recovery. One of the most rewarding aspects of this work is witnessing families reunite and rebuild relationships after substance use has caused deep disruption.

In my private practice, I specialize in SAP (Substance Abuse Professional) evaluations for Department of Transportation (DOT) employees. As a NAADAC-certified SAP, I am committed to providing thorough, fair, and supportive assessments. My goal is to help individuals who have tested positive for substances successfully navigate the return-to-duty process and re-enter safety-sensitive roles with increased awareness, practical tools, and a stronger foundation for lasting wellness.

Outside of work, I enjoy life in Muskegon with my family and two cats, and I find joy in caring for my houseplants. Since 2022, I’ve also had the honor of serving on the board of Life Align, Inc., a nonprofit dedicated to uplifting individuals in recovery.

No matter where someone is on their journey, I believe in their ability to grow, heal, and thrive. If you’re seeking compassionate, evidence-based support through the SAP process, I’m here to help.
